- Your Medicare card
- Any relevant scans or reports
What is a Medicare Chronic Disease Management Plan?
If you're living with a chronic health condition, you may be eligible to receive bulk-billed physiotherapy through a Chronic Disease Management (CDM) Plan (previously known as an Enhanced Primary Care or EPC plan). At Full Foqus Physiotherapy, we proudly bulk bill eligible Medicare physiotherapy appointments, making it easier to access high-quality, evidence-based care without the financial barrier of an out-of-pocket consultation fee. Working closely with your GP, we provide personalised treatment designed to reduce pain, improve movement and help you return to the activities that matter most.

When Medicare covers your physiotherapy
Have a chat with your GP
If you've been managing a health condition for six months or more — or expect to be — your GP can talk through whether a Chronic Disease Management Plan would help.
Your GP prepares the plan
If it's right for you, your GP puts together the Chronic Disease Management Plan and refers you to us for physiotherapy. The number of allied health visits available each calendar year is set by your GP as part of that plan.
Bring your plan and Medicare card
Bring your referral and a valid Medicare card to your first appointment and we'll bulk bill your visit. Not sure where you stand? Speak with your GP, or contact our clinic and we'll guide you through it.
Eligible Medicare CDM referrals are bulk billed — $0 out-of-pocket, every visit
Beyond the cost saving, you get one-on-one consultations with the same physiotherapist, comprehensive assessments, evidence-based physiotherapy, hands-on treatment, progressive rehabilitation and clear communication at every step.
Bulk-billed Medicare physio FAQs
Do I need a referral?
Yes. To receive bulk-billed Medicare physiotherapy, you'll need a valid referral under a Chronic Disease Management Plan from your GP.
Is there an out-of-pocket cost?
No. At Full Foqus Physiotherapy, eligible appointments under a valid Medicare Chronic Disease Management Plan are bulk billed, so there is no gap fee for your approved Medicare physiotherapy sessions.
How many sessions can I receive?
The number of Medicare-funded allied health visits available each calendar year is determined by your GP as part of your Chronic Disease Management Plan.
Can I continue treatment after my Medicare sessions are used?
Absolutely. Many patients continue privately to complete their rehabilitation and maintain their progress. We'll discuss the most appropriate treatment plan based on your goals.
What should I bring to my appointment?
Your GP referral (CDM Plan), your Medicare card, any relevant scans or reports, and comfortable clothing that allows the affected area to be assessed and moved freely.
